Position Description
The coach/case manager will work under the direction of the
Program Coordinator and provide a wide range of case management services
to each project participant for a full year on at least a weekly basis.
Project recruitment will be ongoing throughout the first and partially
into the second year of the program.
Coach/Case Manager in the RESTORE project will be asked to
play a key role serving as advisor, educator, role model and leader
committed to assisting 100 older youth, ages 18 to 24. While also
contributing to the participants learning how to over come their
challenges in life and becoming positive, healthy and fully participating
members of society. This will be based upon the Positive Pathways
approach including, workforce development, coaching/case management and
engaging community to assist in reducing crime, violence and recidivism.
Duties and Responsibilities
Maintains assigned caseload of participants; conducts
intakes, screenings, assesses participant’s needs and resources for the
whole family. Develops and implements case management service plan that
will include monthly, quarterly and yearly goals.
Assist participants in locating and accessing social,
medical, legal, financial, mental health and transportation services.
Assist participants in applying for public entitlement benefits.
Appropriately advocates for participant’s needs.
Facilitates program process groups, family support groups,
education/vocation pathways and budget classes for participants.
Maintains ongoing contact with participants at least once
per week, in person or by phone. Contact other agencies to ensure
effective service delivery. Provide crisis intervention as needed.
Maintains accurate participant records; documents
implementation of service plans; documents all participant contact and
submits monthly reports. Other duties as assigned
Education and Training
Bachelor degree or minimum of two (2) years work
equivalent. Knowledge and experience in substance abuse treatment
preferred, social services required; familiar and comfortable with
recently released male inmates. Must be experienced with
multi-disciplinary team approach. Computer skills necessary.

Responsible for conducting group facilitation services for parolees and
probationers who are referred to and/or, by court order, assigned to
Substance Abuse Aftercare Services.

total group time is three hours. Each group meets approximately 1-2 times
per week.
Qualifications: Licensed as
Professional Counselor, Clinical Social Worker or Marriage and Family
Therapist and/or Addictions Counselor (ICRC, NAADAC, NBCC, GACA). If not
licensed or certified, must be eligible and obtain within 24 months of
hire. Minimum two years of supervised counseling experience. Must have
